How is breast augmentation performed?

Also known as augmentation mammoplasty, breast augmentation surgery involves using implants to fulfill your desire for fuller breasts or to restore breast volume lost after weight reduction or pregnancy. During breast augmentation performed at our practice, we make discrete an incision around the areola, or beneath the breast. The axillary approach is typically selected only upon patient’s request. We continue the breast augmentation procedure by placing the implant either beneath the breast gland or, under the pectoralis muscle. Once you have chosen an incision, the next key decision for your breast augmentation is whether to place the breast implants “under or over the muscle”. The muscle referred to here is the pectoralis major. 

The pectoralis major is a long, wide, triangular-shaped muscle that begins along the entire breastbone and the ribs at the base of the breast and inserts into the humerus at the upper arm. This muscle is a thick and strong layer of tissue that can provide excellent padding over breast implants when needed. This muscle is always required to cover the implants for breast reconstruction in mastectomy patients. In breast augmentation we prefer to place the implant over the muscle if the upper breast soft tissue padding is enough to hide the upper edge (including ripples) of the breast implant. When women have breast tissue that is thin, especially in the upper breast and cleavage areas, there is an increased risk of breast implant visibility in the form of visible ripples, especially over time. Visible ripples in the cleavage area are an obvious sign of an augmented breast. To avoid visible implant rippling it is most often desirable to cover the implants with as much soft tissue as possible. The pectoralis major muscle provides excellent additional soft tissue padding over the breast implants  and we prefer a pocket below pectoralis muscle then.

Variety of implants to choose from

We offer  silicone filled implants of both round and anatomical designs for breast augmentation, including the most advanced "form-stable" silicone implants. If you have enough breast envelope and you just want it to be filled, a round implant may work very well. If your breasts have not developed enough to the desired shape it might be advisable to place an anatomical implant.

What can I expect following breast surgery?

Following surgery, a dressing is placed over the incisions and the breasts are wrapped in a supportive elastic bandage or special surgical bra. There may be a small drainage tube placed in each breast for several days to drain blood and fluid. Our surgeons typically use buried, dissolvable stitches that will not require removal. This technique also results in less visible scars and reduces discomfort at follow-up visits.

    Breast pain in the first several days after surgery may be relieved with medication. Swelling and bruising for several weeks after the surgery is normal, but wearing the surgical bra 24 hours a day will assist to reduce the swelling during the healing process.

    Following two to three weeks (depending on the procedure and the individual patient), you can resume your normal activities. Strenuous exercise or heavy lifting, however, should be avoided for three to four weeks or more. In the meantime, seek out the optimal sports bra for your activity and breast size.
